Knowledge

Everything about Minoxidil

Description

Minoxidil is originally an antihypertensive, developed in the 1960s and approved in the 1980s as a topical solution (Rogaine) for androgenetic alopecia. In recent years, oral low-dose Minoxidil has become a strongly growing research trend in hair restoration.

Per study reports Minoxidil acts as a potassium channel opener with vasodilatory action. The exact mechanisms of hair growth stimulation aren’t fully clarified - improved microcirculation in the follicle area and direct action on anagen phase entry are suspected.

Topical application has been standard for decades. Oral low-dose research (2.5-5mg/day) emerged from the observation that many users with cardiac Minoxidil therapy developed unwanted generalized hair growth. Current research protocols use oral low doses with substantially better adherence than topical application.

A 2022 published study from Brazil (Vano-Galvan et al, JAAD) showed significant hair restoration under 5mg/day oral Minoxidil in androgenetic alopecia.

Pharmacokinetically orally bioavailable with about 4-hour half-life. A once-daily dose of 1.25-5mg is research standard.

Known effects in oral application: hypertrichosis (unwanted facial and body hair), mild tachycardia, water retention. At higher doses blood pressure lowering. Pericardial effects documented at high doses.

You can order Minoxidil as oral tablets, with 2.5mg or 10mg per tablet as a blister of 10 tablets and with 5mg as a bottle of 100 tablets.

Risks & Safety

Minoxidil is originally an antihypertensive. The documented effects of oral low-dose use stem directly from that origin. In the available safety studies, oral low-dose Minoxidil is considered largely well tolerated, with a clearly defined effect profile.

Documented in clinical studies:

  • Hypertrichosis: unwanted hair growth on the face and body is the most commonly documented effect and follows directly from the systemic action.
  • Water retention: fluid retention is described in the literature and is one of the main reasons for slow titration.
  • Tachycardia: mild heart rate increase is documented as a cardiovascular effect of the vasodilatory action.
  • Blood pressure lowering: at higher doses blood pressure falls. At high doses pericardial effects are documented. Low research doses are substantially better tolerated than the original cardiac doses.

This classification does not replace medical advice. Risk and responsibility for any actual use lie entirely with the buyer.

How does minoxidil act on hair?

It is not fully understood. In animal models minoxidil shortens the resting phase, resting follicles switch to growth earlier, the growth phase gets longer and the follicle larger. The active form is the sulfate, which opens potassium channels. Such channels were found in the dermal papilla of human hair follicles. In culture minoxidil and a channel blocker cancelled each other out.

Is minoxidil approved as a tablet?

The compound is: ChEMBL lists minoxidil with phase 4 as the highest phase and first approval in 1979. As a tablet it lowers blood pressure via the vessels. For hair loss the low-dose tablet is used off-label. A phase 3 trial in women is running on the registry, one with tablets under the tongue in men has been completed.

Is the tablet better than the solution applied to the scalp?

In a double-blind trial with 90 men (68 analysed) the tablet was not superior after 24 weeks. The gain in hair density was somewhat higher at the vertex, but not statistically significant. Only the photo assessment put the tablet ahead at the vertex. Its advantage is more about daily life: no daily application. The solution itself is well studied: in a trial with 393 men the higher-concentration solution grew clearly more hair over 48 weeks than the lower-concentration one or placebo.

Why does it not work equally for everyone?

Minoxidil is activated in the hair follicle by the enzyme sulfotransferase. In a study with 41 patients hair loss improved in 63.4 percent after 6 months, more often in men than in women. Surprisingly, patients with low enzyme activity responded better (85 against 43 percent). The authors see this as a way to estimate beforehand who benefits.

Which side effects were seen?

In the largest analysis with 1,404 patients increased body hair was most common (15.1 percent). Lightheadedness (1.7), fluid retention (1.3), racing heart (0.9), headache (0.4), swelling around the eyes (0.3) and sleep problems (0.2 percent) were rare, 1.7 percent stopped. In a small trial in men almost half had more body hair.

Does it also help with diffuse shedding?

In chronic telogen effluvium, long-lasting diffuse shedding without bald patches, it was studied in 36 women. The shedding score fell from 5.64 by 1.7 points after 6 months and by 2.58 points after 12 months. Blood pressure barely changed, 13 women developed more facial hair. It was a retrospective analysis without placebo.

How good is the overall evidence?

A review found 17 studies with 634 patients in total up to April 2020, mostly on pattern hair loss, plus other forms. It rates the tablet as effective and well tolerated for healthy people who struggle with the solution. According to the authors larger randomised trials comparing doses are still missing.
