Everything about Alprostadil

Description

Alprostadil (Prostaglandin E1, PGE1) is a synthetic prostaglandin approved in multiple clinical indications: intracavernous application for erectile dysfunction (Caverject), urethral application (MUSE) and in pediatric cardiology to maintain patent ductus arteriosus.

Per study reports Alprostadil binds to prostaglandin EP receptors in vascular smooth muscle. In the penile corpus cavernosum activation leads to vasodilation of trabecular arterioles and to smooth muscle relaxation - with consequent erection promotion.

Compared to PDE-5 inhibitors (Tadalafil, Sildenafil), Alprostadil has two important differences: first, local application form (intracavernous or intra-urethral) instead of oral; second, action independent of sexual stimulation. This makes Alprostadil a tool for PDE-5 non-responders or for research setups with completely neural ED mechanism.

Pharmacokinetically very short - after injection into corpus cavernosum half-life of a few minutes. Action begins within 5-10 minutes and lasts 30-60 minutes.

Known effects: local pain at injection site (in the majority of users), more rarely priapism (prolonged erection over 4 hours). At very high doses systemic vasodilation with blood pressure drop.

You can order Alprostadil as lyophilized peptide in 5-pack at 20mcg per vial.

Risks & Safety

Alprostadil is a prostaglandin approved in multiple clinical indications and is therefore well characterized, but as a vasodilator with local application it shows a defined risk profile.

Documented in clinical studies and the literature:

  • Local pain: pain at the injection site is among the most common observations with intracavernous application according to the literature.
  • Priapism: a prolonged erection over several hours is a documented risk, particularly at higher dosages. It is regarded as a serious complication.
  • Hypotension: at higher doses the vasodilatory effect can become systemic and trigger a blood pressure drop.
  • Dangerous combinations: in parallel with PDE-5 inhibitors such as tadalafil or sildenafil, additive vasodilation is documented.
  • Local reactions: beyond pain, the literature describes tissue reactions with repeated application at the same site.

This classification does not replace medical advice. Risk and responsibility for any actual use lie entirely with the buyer.

Still have a question?

Write to us, our team will help you.

Is alprostadil approved?

Yes. The compound has been approved in the USA since 1981. As an injection into the erectile tissue it is approved for erectile dysfunction and as an aid in diagnosis. Another approved use is in newborns to keep a blood vessel at the heart open.

How does alprostadil work?

Alprostadil binds to prostaglandin receptors in the erectile tissue and raises the messenger cAMP inside the cells. The smooth muscle relaxes and the arteries widen. Blood flows in and is held in the erectile tissue.

What do the studies show?

In a study of 683 men who injected themselves for six months, 94 percent of injections were followed by sexual activity. Men and partners were satisfied after 87 and 86 percent. Given into the urethra, 64.9 percent of men had intercourse at home versus 18.6 percent with placebo.

Which side effects are known?

Most common is penile pain: 37 percent in the approval studies versus 2 percent with placebo, mostly mild to moderate. There were also prolonged erections (4 percent), hardening (3 percent) and bruising (3 percent). Given into the urethra, blood pressure dropped in 3.3 percent in the clinic.

What is priapism?

An erection that lasts longer than 6 hours. In the studies it occurred in 0.4 percent. The prescribing information says: if an erection lasts longer than 4 hours, get medical help immediately. Untreated, the tissue can be damaged permanently.

Who should not use alprostadil?

According to the prescribing information not with a tendency to priapism (such as sickle cell disease, multiple myeloma or leukaemia), with hardening or curvature of the penis, with a penile implant or with hypersensitivity to the compound.

Do spontaneous erections come back?

In an open study of men with circulation-related erectile dysfunction, 85 percent of 54 men with data reported during a year of self-injection that spontaneous erections had become more frequent. Blood flow in the erectile arteries had increased. There was no comparison group.
