Botulinumtoxin A (BTX-A)

BTX-A is botulinum toxin type A, a neurotoxin. Temporarily blocks signal transmission from nerve to muscle and is one of the longest-studied compounds in aesthetic research.

Description

BTX-A is botulinum toxin type A, a large protein produced by the bacterium Clostridium botulinum. In research it is one of the longest-used tools for interrupting the transmission between nerve and muscle in a targeted and temporary way.

The mechanism is well described: the toxin binds to the endings of motor nerves, is taken up there and cleaves the protein SNAP-25. Without intact SNAP-25 the nerve endings can no longer release acetylcholine. The targeted muscle stays relaxed. The effect stays limited to the area around the application site and lasts until the nerve endings have formed new connections, typically several months in studies.

BTX-A has been studied most intensively in aesthetic research. The first observations on expression lines between the eyebrows date from 1992. Large placebo-controlled studies followed in the early 2000s. There is also a broad literature on neurological questions.

Potency is not stated in milligrams but in International Units (IU). These units are defined differently for each preparation and cannot be transferred one to one from one preparation to another.

You can order BTX-A as lyophilized powder in 10-vial packs with 100 IU per vial.

Risks & Safety

BTX-A is one of the most potent biological toxins known. Even the smallest amounts are active. The gap between the amounts used in studies and a dangerous amount is small. Its classification is therefore stricter than for almost any other product in the range.

Documented in studies, adverse event reports and case reports:

  • Spread beyond the application site: the toxin can move away from where it was applied. Reported effects include drooping eyelids, double vision, dry mouth, difficulty swallowing and speaking, muscle weakness and shortness of breath, sometimes only days after application.
  • Use outside medical settings: in a 2024 report by the US health agency CDC, seven women fell ill after such applications, four were hospitalized and two of them needed intensive care. In at least four cases the product was counterfeit.
  • Units are not interchangeable: the IU values of different preparations are defined differently. Transferring values from studies can be off by a multiple.
  • Local effects: headaches and temporarily drooping eyelids were the main observations in the controlled studies.
  • Exclusions in the literature: disorders of neuromuscular transmission such as myasthenia gravis, pregnancy and breastfeeding.
  • Loss of effect with frequent use: repeated applications at short intervals can trigger neutralizing antibodies, after which the effect wears off.

Risk tier 3, the strictest in the range. In the literature every application is performed by trained medical staff. This classification does not replace medical advice. Risk and responsibility for any actual use lie entirely with the buyer.

Still have a question?

Write to us, our team will help you.

Is botulinum toxin A approved?

Yes, as a medicine in the US since 1989, first for strabismus and eyelid spasm, for frown lines since 2002. Each product has its own approval and its own units. This lexicon deliberately gives no dosing.

How exactly does BTX-A work?

The compound is a zinc-dependent enzyme. In the nerve ending it cleaves the protein SNAP-25 near its end, as shown in Nature in 1993. Without intact SNAP-25 the ending cannot release acetylcholine and the muscle stays relaxed. The nerve endings recover over weeks to months.

What did the large placebo-controlled study show?

264 people with moderate to severe frown lines took part, 203 received the compound, 61 placebo. The lines receded more than on placebo at every visit, for many this lasted to day 120. Drooping eyelids occurred in 5.4 percent, mostly mild.

Does the effect wane with repeated treatment?

In a study with 796 participants and up to eight treatments at least 85 days apart, response 30 days after each treatment was between 79 and 90 percent. Shortly before the next treatment the effect had mostly faded. The regular entry names neutralising antibodies after treatments at short intervals as a possible cause of loss of effect.

What do the reports to the US agency show?

1,437 reports were analysed. Serious events mostly followed medical use with higher amounts, all 28 deaths fell into this group. After cosmetic use the most common reports were lack of effect, injection site reactions and drooping eyelids.

Why is use outside medical practices so risky?

A report by the US health agency describes seven women who fell ill in 2024 after use without medical staff, among other things with drooping eyelids, difficulty swallowing and shortness of breath. Four were hospitalised, two of them in intensive care. At least four had received counterfeit product.

How long does BTX-A keep after mixing?

The prescribing information requires prompt use, usually within 24 hours refrigerated. In a double-blind study vials mixed up to six weeks ahead and kept refrigerated worked just as well. An expert consensus considers refrigerated storage of at least four weeks acceptable for facial use. The rule for our peptides does not apply here.
