Description

Methylene Blue (Methylthioninium chloride) is a classical phenothiazine dye used in medicine since 1891 - as antidote for methemoglobinemia Methylene Blue is clinically approved and on the WHO list of essential medicines.

Per study reports Methylene Blue shows broad biological activity beyond clinical indication: improvement of mitochondrial respiratory chain efficiency through direct electron carrier function, neuroprotective effects in Alzheimer and Parkinson models, monoamine oxidase inhibition with potential mood effects, antimicrobial action.

In the recreational community Methylene Blue is a mitochondrial compound used at low doses (0.5-2mg/kg). Higher doses aren’t recommended - Methylene Blue has a biphasic dose-response curve (hormesis-like) where low doses show positive effects and higher doses become toxic.

Pharmacokinetically orally bioavailable with good bioavailability, half-life about 5-6 hours. Visible staining of urine and partially mucous membranes is known.

Known effects: at correct dosing very well tolerated. At higher doses methemoglobinemia risk (paradoxical effect). Not to be combined with serotonergic active substances (SSRIs, MAO inhibitors) - serotonin syndrome risk documented.

Methylene Blue is described as well tolerated per study reports at low dosages, but as an orally bioavailable phenothiazine dye it has relevant interactions and contraindications that set it apart from pure peptide compounds.

Documented in research and the literature:

  • Serotonergic interactions: Methylene Blue acts as a monoamine oxidase inhibitor, and serotonin syndrome is documented in combination with serotonergic substances such as SSRIs or MAO inhibitors. This combination is considered a serious interaction.
  • Biphasic dose-response: Methylene Blue shows a hormesis-like curve, where low doses are associated with positive effects and higher doses become toxic and carry a paradoxical methemoglobinemia risk.
  • G6PD deficiency: in glucose-6-phosphate dehydrogenase deficiency, Methylene Blue is contraindicated in the literature, as it can trigger a hemolytic reaction.
  • Discoloration: a visible blue staining of urine and partially mucous membranes is known and harmless.
  • Limited long-term data: the clinical use as an antidote is established, but robust long-term data on low-dose research use are limited.
