Description

Orlistat differs from almost everything else in weight regulation in that it never really enters the body. The substance inhibits gastric and pancreatic lipases directly in the intestinal lumen. Those enzymes split dietary fat into absorbable fragments; without them a portion of the fat stays undigested and is excreted. The review by Heck and colleagues describes the mechanism as purely local, with only a very small fraction absorbed systemically.

The entire profile follows from that. There is no central nervous effect and no interference with appetite, drive or heart rate. Consequently there are none of the problems earlier appetite suppressants had. The adverse effects are instead all gastrointestinal and depend directly on how much fat is eaten. Eat a high-fat meal and you get them in full; reduce the fat and you barely notice them.

The effect size deserves a sober look. Heck and colleagues summarise that orlistat outperformed diet alone across trials lasting up to two years, while remaining a modest effect, accompanied by moderate improvements in total cholesterol, LDL, blood pressure as well as fasting glucose and insulin. The review by Finer adds the XENDOS data: across four years the additional, if modest, weight loss on orlistat delayed the development of type 2 diabetes.

One point has practical consequences that are easily missed. Because fat is not fully absorbed, uptake of fat-soluble vitamins drops too. Heck and colleagues therefore explicitly recommend a standard multivitamin during use, taken at a different time of day.

In a research context orlistat is therefore a peripherally acting lipase inhibitor whose effect depends entirely on the fat content of the diet. It is not comparable to centrally acting substances.

You can order Orlistat as oral tablets with 120mg per tablet, 10 tablets per blister.

Risks & Safety

Orlistat has an unusually harmless systemic profile and a very present local one. Anyone who has understood the mechanism can judge both well.

  • Gastrointestinal effects are the mechanism, not a side effect: Oily or fatty stools, urgency, flatulence with discharge and, in unfavourable cases, faecal incontinence. Heck and colleagues note that these effects occur early and tend to decrease with continued treatment, because most users adjust their fat intake. A single high-fat meal can bring the symptoms back at any time.
  • Fat-soluble vitamins: Uptake of vitamins A, D, E and K falls. A multivitamin taken at a different time of day is the explicit recommendation of the literature, not an optional extra.
  • Drug interactions: Substances whose absorption depends on fat are affected. This is relevant for ciclosporin, levothyroxine, antiepileptics and vitamin K antagonists among others. Spacing out doses helps only partially here.
  • Modest effect size: Heck and colleagues themselves point out that the modest weight loss and the extent of gastrointestinal discomfort may limit practical utility. Anyone looking for a substance that works without dietary change will not find it here.
  • Liver and kidney: Rare reports of severe liver injury and of oxalate nephropathy are documented. Both are rare but belong in the picture.

This classification does not replace medical advice. Risk and responsibility for any actual use lie entirely with the buyer.

Studies

  • Orlistat, a new lipase inhibitor for the management of obesity - Pharmacotherapy 2000, Heck et al: review of the local lipase inhibition mechanism, trials lasting up to two years and effects on cholesterol, blood pressure and fasting glucose, plus the recommendation for a multivitamin supplement.
  • Pharmacotherapy of obesity - Best Practice & Research Clinical Endocrinology & Metabolism 2002, Finer: classification of orlistat and sibutramine, including the four-year XENDOS data and the delayed onset of diabetes.
