Cock Bombs is a combination tablet of 50mg Sildenafil and 20mg Tadalafil. The combination is not clinically approved in any country; in the recreational community it’s used because of the complementary pharmacokinetics of the two PDE-5 inhibitors.
Per study reports the tablet combines two action profiles: Sildenafil with fast onset (30 min) and 3-5 hour half-life, plus Tadalafil with slower onset (45-60 min) and 17.5-hour half-life. Practically this yields an action profile with rapid start (Sildenafil-driven) and long taper (Tadalafil-driven) over 24-36 hours.
Mechanistically the two PDE-5 inhibitors don’t amplify each other synergistically but additively - both block the same enzyme. Effects on cGMP concentration in corpus cavernosum muscle are the same as with single compound, only the temporal profile is mixed.
Safety profile: the combination adds the side effects of both compounds, so headache, facial flushing, and nasal congestion are more frequent than under a single compound. Clinical data on chronic application of the combination is absent.
The tablet is more effective fasting (Sildenafil portion sensitive to fatty meals); in practice the community doses 30-60 min before activity.

- Nitrate interaction: combination with nitrates or NO donors is contraindicated - both compounds act via the same pathway, and the combo does not make the effect safer.
