Testosterone Undecanoate is the longest-acting of common test esters, formed by esterification at the 17β-hydroxyl with undecanoic acid. The eleven-carbon side chain makes the compound extremely lipophilic and is therefore released via the intestinal lymphatic pathway (oral form) or very slowly from the intramuscular depot.
Per study reports the IM elimination half-life is about 20.9 days in tea seed oil and 33.9 days in castor oil. This makes Undecanoate the only test ester that works clinically with 10-14 week injection intervals. The long intervals produce a remarkably smooth plasma level profile without the typical roller-coaster effects of shorter esters.
Pharmacokinetic studies show that Undecanoate tends to control aromatization to estradiol better through the lower peak values than high-dose short-acting esters. Compared to transdermal gel formulations, IM Undecanoate delivers more stable steady-state concentrations.
In TRT research literature Undecanoate is considered the gold standard for patients with adherence issues or who prefer low injection frequency. In bodybuilding research the substance is less common because the long half-life makes mid-protocol adjustments nearly impossible.

Risks & Safety
Testosterone Undecanoate shares the risk profile of all testosterone esters - the lower peak values tend to control aromatization better, but the long half-life makes clearance take months if side effects occur.
Documented in studies and the application literature:
- Estrogen conversion: testosterone aromatizes to estradiol. Through the lower peak values the E2 rise tends to be more controllable in studies than with short-acting esters, yet water retention and blood pressure rise are documented.
- Suppression of endogenous production: exogenous testosterone throttles the body’s own production via the HPTA axis. Testicular atrophy and reduced fertility are documented, recovery after discontinuation is not guaranteed.
- Cardiovascular: studies describe unfavorable shifts in the lipid profile (HDL falls), elevated hematocrit and an associated thrombosis risk.
- Long half-life as a risk: if side effects occur, the compound cannot be quickly dialed back because of the 20-34 day half-life. Clearance takes months.
