Everything about LL-37

Description

LL-37 is the only human cathelicidin and the primary antimicrobial peptide of innate immunity in humans. Structurally it is a 37-amino-acid peptide released from the precursor protein hCAP-18 after proteolytic cleavage. The name “LL-37” derives from the two N-terminal leucine residues and the total length of 37 amino acids.

The research mechanism is multilayered. The central activity lies in direct antimicrobial action against bacteria, viruses and fungi. The cationic, amphipathic structure of LL-37 allows interaction with negatively charged microbial membranes and leads to membrane permeabilization. In preclinical studies, this broad antimicrobial spectrum has been documented.

Beyond direct antimicrobial activity, LL-37 shows immune-modulatory effects in research models: leukocyte chemotaxis, modulation of cytokine production, promotion of wound healing and angiogenic properties. This multi-modality makes LL-37 a mechanistically complex but well-characterized research tool at the interface between innate immunity and repair.

In preclinical studies, LL-37 has been used in wound healing models, skin research setups and inflammation studies. A particularity: LL-37 levels are altered in some chronic inflammation states, also making the compound relevant in pathophysiological research.

LL-37 is available in 5mg vials as 10-vial packs.

Risks & Safety

LL-37 is described as largely well tolerated per study reports. As an endogenous antimicrobial peptide of innate immunity, the compound is mechanistically well characterized.

Observed in research and user reports:

  • Injection site: mild redness or irritation at the injection site is the most common local observation, typical for subcutaneously applied peptides.
  • Membrane activity: the cationic, amphipathic structure of LL-37 permeabilizes microbial membranes. Local irritation is plausible as an expression of this activity.
  • Immune modulation: beyond its antimicrobial action, LL-37 modulates chemotaxis and cytokine production, an active intervention in innate immunity. Altered LL-37 levels are documented in some chronic inflammation states.
  • No long-term data: the characterization comes predominantly from preclinical models. Robust long-term human data do not exist.

This classification does not replace medical advice. Risk and responsibility for any actual use lie entirely with the buyer.

Still have a question?

Write to us, our team will help you.

Is LL-37 approved or studied in humans?

Not approved, but tested in humans. Only locally though: applied to hard-to-heal leg ulcers (one trial with 34 and one with 148 participants) and as an injection into melanoma metastases (4 participants). There is no trial with LL-37 injected under the skin. The information here describes the state of research.

Where does LL-37 come from in the body?

It is cleaved from the precursor hCAP18. Immune cells make it all the time, skin and mucous membranes mainly during inflammation and infection. In wounds the level rises until about 48 hours after the injury and falls again once the wound has closed. Immune cells also make it depending on vitamin D.

How does LL-37 work?

In two ways. It attaches to the membrane of bacteria and breaks it open. It also attracts immune cells such as neutrophils, monocytes and T cells, via the receptor FPR2 in cell studies. In a skin model it helps wounds close with new skin.

What did the wound trials show?

In the first trial with 34 participants leg ulcers healed about six and three times faster than with placebo at the two lower concentrations, the mean area fell by 68 and 50 percent. The highest concentration worked no better than placebo. The larger phase 2b trial with 148 participants found no overall difference, only a post hoc analysis of large wounds favoured LL-37.

Can LL-37 increase inflammation?

Yes, that has been described. In rosacea cathelicidin is strongly raised in the facial skin and cut differently, in mice these fragments triggered inflammation. In psoriasis LL-37 binds the body's own DNA and makes it visible to certain immune cells, which then release interferon. The entry points this out too.

Which side effects are known?

In both wound trials LL-37 was well tolerated, there were no safety concerns at the wound or in the body. There are no study data for LL-37 injected under the skin. The entry mentions possible reactions at the injection site because LL-37 attacks cell membranes.
