What is Modalert?
Modalert is the brand name under which Sun Pharmaceutical Industries sells modafinil in 200mg tablets. The active substance is the same as in the unbranded generic: racemic modafinil, a 1:1 mix of the R- and S-enantiomers, at the same strength and with the same effect profile. What changes is not the pharmacology but the provenance. Sun Pharma is India's largest pharmaceutical company and one of the largest generic manufacturers worldwide. Modalert has been one of the internationally best-known modafinil brands for years.
Modafinil itself belongs to the eugeroics, the wakefulness promoters. It is neither structurally nor pharmacologically an amphetamine derivative. The subjective state is closer to "not tired" than to "wired". The substance is approved for excessive daytime sleepiness in narcolepsy, obstructive sleep apnea and shift work disorder. Everything the lexicon says about modafinil applies to Modalert one to one. This entry adds what a brand does and does not deliver compared with a generic.

How it works
The mechanism of modafinil is still not resolved in every detail, but the outline is well documented. At its core sits inhibition of the dopamine transporter: modafinil blocks dopamine reuptake into the neuron and thereby raises extracellular dopamine. Unlike amphetamines it does not push dopamine out of the storage vesicles. On top of that, cortical catecholamines rise, orexinergic, histaminergic, serotonergic and glutamatergic systems are indirectly stimulated and GABA falls, as Battleday and Brem summarize in their 2015 review. Those are the circuits that run the sleep-wake cycle, not the general drive systems.
How different that feels from a classic stimulant was measured by Jasinski (Journal of Psychopharmacology 2000) in 24 men with a history of cocaine use: modafinil at 200 to 800mg was distinguished from placebo and the participants liked its effects, but it produced no significant response on the amphetamine scale, suppressed sleep more than methylphenidate, raised standing heart rate less and cut appetite less. Pharmacologically, Modalert is a wakefulness promoter, not an upper.
Brand or generic: what changes and what does not
A generic is only allowed on the market if it is bioequivalent to the reference product. The usual yardstick: the 90 percent confidence interval for area under the curve and peak concentration has to fall within 80 to 125 percent of the original. Pharmacologically, brand and generic are therefore interchangeable. Anyone expecting a stronger or longer effect from Modalert than from generic modafinil 200mg is expecting something that does not exist.
What the brand delivers sits on a different level: a manufacturer known by name with its own quality control, traceable batch numbers and a production line that turns out the same product year after year. Excipients, tablet shape and disintegration can differ between manufacturers without breaking bioequivalence. For the research question of how closely one batch matches the next, that is exactly what counts. For effect, half-life and risks, it does not.
Put plainly: if you only want the substance, the generic is the cheaper route. Modalert is for those who specifically want the brand and the manufacturer behind it.
Pharmacokinetics: why timing counts
After oral dosing the peak plasma level is reached after 2 to 4 hours; a meal pushes it back by about an hour without changing total absorption (prescribing information). Wong and colleagues (Journal of Clinical Pharmacology 1999) gave healthy men 200, 400, 600 or 800mg daily for seven days: steady state was reached after three days, the half-life sat around 15 hours. The kinetics were linear across the whole range. The S-enantiomer was cleared about three times faster than the R-enantiomer, which is why the plasma level of racemic modafinil declines in two phases. The 800mg group was stopped after three days because blood pressure and pulse were rising.
In practice the half-life is the number that matters. A tablet at 1pm still carries roughly half of its peak level at midnight. That is why the same rule applies to Modalert as to any modafinil: early in the morning or not at all. Elimination runs mainly through the liver; in severe hepatic impairment the prescribing information calls for halving the dose. Exposure is higher in older adults.
Use and dosing
What the research shows
The strongest evidence concerns sleepiness that is not self-inflicted. Czeisler and colleagues (NEJM 2005) randomized 209 patients with shift work disorder to 200mg modafinil or placebo before each night shift for three months. Nighttime sleep latency rose by 1.7 minutes on modafinil versus 0.3 on placebo, 74 instead of 36 percent reported clinical improvement, lapses of attention on the vigilance test decreased. On the way home, 29 instead of 54 percent had an accident or near accident. The authors still note that treated patients remained markedly sleepy: modafinil takes off the peak, not the problem.
In healthy people under sleep deprivation, Wesensten and colleagues (Psychopharmacology 2002) arrive at a sobering comparison: 50 young adults stayed awake for 54.5 hours and after 41.5 hours received placebo, modafinil 100, 200 or 400mg, or caffeine 600mg. Modafinil 200 and 400mg maintained performance and alertness through the night, but no better than the caffeine dose. The two modafinil doses did not differ significantly. The authors' conclusion: for otherwise healthy adults under sleep loss, modafinil offers no advantage over caffeine.
On cognitive enhancement without sleep deprivation the picture is mixed. Battleday and Brem (European Neuropsychopharmacology 2015) reviewed every study from 1990 to 2014 in rested healthy adults: in basic testing paradigms modafinil mostly improved executive function, improved attention and memory in only about half of the studies. A few studies even found worse divergent, meaning creative, thinking. With more complex tasks the gains were more consistent. That is not an intelligence booster, more a drug that supports stamina and focus during long, demanding tasks. There are no separate studies of Modalert as a brand; the evidence is that of the active substance.
Interactions: CYP3A4 and hormonal contraception
Modafinil induces CYP3A4 and inhibits CYP2C19. To a lesser degree it induces CYP1A2 and CYP2B6 (prescribing information). The most relevant consequence concerns hormonal contraceptives: their active substances are cleared through CYP3A4. The prescribing information states that hormonal contraception is less reliable during use and for about one month afterwards. The same applies to other CYP3A4 substrates such as cyclosporine, midazolam or triazolam, whose levels fall.
In the other direction, levels of CYP2C19 substrates such as diazepam, omeprazole, propranolol or phenytoin rise. For warfarin the prescribing information recommends closer monitoring of coagulation. Anyone using one of these substances should take that question to a doctor. That is as true for Modalert as for any other modafinil.
Side effects
- Serious skin reactions: rare but serious. Rash requiring hospitalization, up to Stevens-Johnson syndrome and toxic epidermal necrolysis, is documented for modafinil, along with multi-organ hypersensitivity (DRESS) and angioedema, mostly within the first one to five weeks. Any new rash, any blistering and any involvement of mouth, eyes or mucous membranes is a reason to stop and a case for medical assessment.
- Headache: by far the most common and dose-dependent, about 34 versus 23 percent on placebo in the pooled registration trials.
- Nausea, nervousness, anxiety, dizziness, dry mouth, reduced appetite: regularly reported, mostly mild to moderate. Nausea ran at about 11 versus 3 percent.
- Insomnia: about 5 versus 1 percent, almost always a question of timing rather than dose.
- Circulation: mild increases in blood pressure and heart rate. In the dose study by Wong and colleagues, exactly that ended the 800mg arm. Anyone with blood pressure issues should measure along. With left ventricular hypertrophy, or a mitral valve prolapse that has already caused symptoms under stimulants, the prescribing information advises against modafinil altogether.
- Mental state: anxiety, agitation, irritability and in isolated cases psychotic or manic symptoms are described, particularly with a prior history. The prescribing information calls for particular caution there.
- Abuse potential: lower than with amphetamines, but not zero. In Jasinski's study experienced users liked the effect without it being amphetamine-like. In the US modafinil is a Schedule IV controlled substance.
- Pregnancy: registry data from the US and Denmark showed clearly more malformations than expected after first-trimester modafinil (around 12 to 13 instead of about 4 percent), while a Swedish-Norwegian cohort did not find the signal. Regulators in the EU and US have advised against modafinil in pregnancy since 2019. Together with the weaker hormonal contraception, that is a double issue.
Storage
Store dry, at room temperature and protected from light. Tablets are best left in the blister until you need them, out of reach of children. Use a split tablet promptly rather than leaving the other half lying open for weeks.
